From the Health Office - To the Parent(s)/Guardians of 6th Grade Students

As a reminder: Nebraska State law states that your student must have the following to attend 7th gr.:

3 DPT/Td - one, which must be after the age of 4.  It is also recommended that they have an updated Tdap before 7th grade

3 Polio

2 MMR - both must be after the age of 1 year

3 Hepatitis B

**Varicella (chicken pox) - either the shot or a date for this disease

If you have questions about your student's immunization dates and whether they meet the state law, feel free to call 436-1211 and ask for the health office.
